Step 1: Create Your Patient Portal

3 - 5 Minutes

Please click here to create your portal. You will need to do this to access the necessary forms for your health assessment!

Step 2: Submit Your Paperwork

25 - 45 Minutes

You have two forms to complete: Initial Paperwork and HIPAA Consent.

• Once logged into your portal, go to Forms on the right-hand menu.

• You will see both forms as an option to complete. Select the Start button beside each form to begin filling it in.

• Once a form is complete, ensure to hit Submit at the bottom of the page to finalize your submission.

Helpful Notes:

• to Save, select Save and Continue on the upper right-hand corner.

• to continue where you left off, select the Continue from Previous button beside your saved form.

Step 4: Including Your Significant Other

Do you have a spouse or significant other? If so, get them involved and have them at your appointment, whether it is virtual or in-person.

Leading up to your appointment, discuss together what you are looking to achieve with Naples Vitality.

Step 5: Getting To Your Appointment

(Virtual)

10 - 15 Minutes

1. Visit the Zoom Website

Click HERE to be taken to Zoom

2. Install Zoom

Select "Install" if you are asked to install Zoom

3. Test (OPTIONAL)

Use this Zoom link to ensure that you are able to join a meeting

All set! You can close out of Zoom when you are done. For your appointment, you'll only have to click the link that you will receive by email 1 day ahead of your appointment.
